Supplemental Forms for Investigators

Concept of Operation:
  1. Supplemental Forms are created in your Account Settings (menu with your name., Account Settings, Supplemental Forms tab)
  2. For every form you create, the is a checkbox labeled "Display on Questionnaires. When checked, the form will be presented to all references upon login. The intent is to display a consent authorization form, but you can designate other forms if needed.
  3. When a case is created, the forms related to the position assigned for the case are copied to the case. No changes to the existing supplemental forms library are cascaded to existing cases, only to cases created after the change. 
  4. If you change the position assigned to a case, POBITS will delete all existing supplemental forms for the case, look up the position, and replace them with the supplemental forms assigned to the selected position. Therefore, it is important to assign all cases to a position.
  5. To relate supplemental forms to a position, open or create a position and select the supplemental forms you want to apply.
