Reference Questionnaire

As a reference to the candidate, you may be a relative, a personal/professional friend or acquaintance, a previous or current employer, a previous or current landlord, or a law enforcement agency to which this candidate has applied. You questionnaire will be different depending on your relationship to the candidate. Regardless of your relationship, the basic steps are the same and will take no more than a few minutes of your time. 

Start by viewing the candidate's consent authorization release if provided. This form, signed by the candidate, authorizes parties to release personal and professional information for the purpose of this investigation. 

Any information you provide in the questionnaire is strictly confidential.

Next, open the questionnaire which will have 10-15 questions. Answer all questions honestly and provide information only where you have first-hand knowledge. You must provide some answer to each question. If you need more time, scroll to the bottom of the form and press Save, then Close and you can sign out and return later to complete it. When finished, press Save, then press the Submit button at the bottom of the form. This process will validate your responses and log you out of the system, after which your access to the system is revoked. You should receive an email thanking your for you participation.
