Preliminary Questionnaire

The Preliminary Questionnaire option is currently only available when using the Modern version of the PHS.
When the Preliminary Questionnaire attribute is checked, The PHS only display and validates Yes/No questions and does not display ancillary information to the candidate or investigator. Using a preliminary questionnaire is useful for providing an abbreviated questionnaire. The candidate does not provide information on relatives, references, employers, or landlords and several other items. This feature is provided for a large organization that gains benefit from it. In general, we do not recommend using it. The specifics are as follows:


  1. Supplemental forms are not affected.
  2. The Relatives investigative area is not displayed or validated.
  3. The References investigative area is not displayed or validated.
  4. For the Residence investigative area, the candidate is asked all questions but is not required to provide the residence history.
  5. For the Education investigative area, the candidate is asked all questions but it not required to list any schools or academies.
  6. For the Employment investigative area, the candidate asked all questions but it not required to list any employment. 
  7. The Military investigative area is not affected.
  8. For the Financial investigative area, the candidate is not required to list Income, Expenses, Assets, or Liabilities.
  9. The Legal investigative area is not affected.
  10. The Sexual Conduct investigative area is not affected.
  11. The Social Media investigative area is not displayed or validated.
  12. The Drug Use investigative area is not affected.
  13. For the Motor Vehicle Operation investigative area, the candidate is not required to provide information on vehicle licenses or insurance.
  14. The Other Topics investigative area is not affected.
