This section requests your name, citizenship, address, personal and work contacts, and other data such as your SSN, place of birth, and driver's license. Hover over the the various question marks next to the items for addition information. Some fields are required but may not be be marked with a * symbol. However, any missing or unacceptable inputs will be reported when you attempt to save. Generally, all information is required.
Note that laws in across states vary in terms of what you are required to respond to with regard to legal authorization to work. This section of the form follows the California model in which responses for unsworn positions are not required to answer. If you are applying for a sworn position i.e., a police officer position, you are required to answer that question.
