Personal History Statement
The Personal History Statement (PHS), sometimes called a Personal History Questionnaire (PHQ), probes a number of different areas of your personal history. This help topic focuses on how the forms work and does not refer to any specific question or topic area.
- The buttons at the top of the screen navigate between the topic areas. Each topic area is different in its content. From the main screen, you can launch any single topic.
- Most of the areas contain lists for you to add information, such as a relative, reference, prior employer, etc. Use the Add New button to add items to the list. Once a list item is added, you can select it to edit it or use the Delete link associated with the item to delete it altogether.
- Once you complete all section, we recommend you use the Validate button to validate the content. This will inform you if there are any identifiable errors.
- Once your form passes all validation, use the Submit button to submit it. Once the form is successfully submitted, you will be asked to log out and after that, your access to the system is terminated.
