Custom Questions

Within your account, you can customize all numbered questions for each investigative area. Here is what you need to know:
  1. Custom Questions only apply to what is called the "Modern" version of the PHS.
  2. The default questions are based on the State of Oregon's most recent questionnaire as of 2023.
  3. Every time a case is created, the questions from this area are copied into the case. Therefore, changes to these questions have no effect on other cases — only to those created after modifying the questions. The benefit is that completed PHSs remain unchanged and will always present the questions as asked at the time of response.
  4. You cannot add more questions than already exist.
  5. If there are any questions you do not want to use, change the question to say something like "This question is discontinued, please answer No". At this time, questions cannot be deleted or hidden from the questionnaire, but this method will work perfectly.
  6. All questions are the same format i.e., a Yes/No question is asked, and a Yes/No response is required. Any question with a Yes response, requires an explanation in that section.
  7. We suggest you review each question carefully to ensure it meets your requirements.
