Completing the PHS

Completing the PHS

You can save your work at any time and return later. However, you have 10 days from the date your last credentials were issued before they expire. If they do expire, only your investigator can issue new credentials.

Once you have completed the entire questionnaire, you must submit it. This process validates that all questions are answered and explained. Once your PHS has been submitted, your system credentials are revoked.
